Support Coordination
Level 1 & Level 2 Support Coordination
Phoenix – Mental Health Australia provides Level 1 and Level 2 support coordination, helping you understand your NDIS plan, connect with services and build confidence in managing your supports. We work alongside you, with your goals and choices guiding the process.
Level 1 – Support Connection
Practical help to understand your plan and connect with NDIS providers, community services and mainstream supports. We help you get started and build the skills to organise your supports.
Level 2 – Coordination of Supports
More involved help to put a mix of supports in place, coordinate providers and address barriers or gaps in services. We help you build the skills to maintain relationships, manage daily tasks, live more independently and participate in your community.
What does a support coordinator do?
• Explain your plan, funded supports and available options.
• Help you compare and choose providers and understand service agreements.
• Coordinate services and communicate with your support network, with your consent.
• Help you monitor how supports and funding are being used.
• Address service difficulties and plan for changes in your needs.
• Prepare progress reports and support you to gather information for plan reassessments.
• Build your confidence to make decisions and manage your own supports over time.
Your choice and control
You choose your providers. Any potential conflicts of interest are discussed openly, including when Phoenix also provides other supports.
